Job summary
Reputable Community Healthcare Clinic in California is seeking a Community Health Worker/Enhanced Care Management (ECM) Care Coordinator to reach out to the community, educate patients, and help them access health insurance, healthcare services, and needed supports.
You will coordinate care for patients with complex medical and social needs, work with primary care and behavioral health teams, address barriers to care, and connect clients to housing, transportation, and other community resources.
Qualifications
- Community Health Worker certification is required.
- High school diploma or GED; or equivalent experience.
- Fully bilingual in Spanish and English.
- Experience or training helping people and connecting them to resources.
Responsibilities
- Coordinate care for patients with complex needs and ensure access to services.
- Develop and monitor individualized care plans.
- Collaborate with primary care and behavioral health teams.
- Advocate for housing, food, transportation, and mental health resources.
- Document care interactions and maintain records; ensure EHR compliance.
- Engage in outreach to high-risk patients and quality improvement.
