Hometown Urgent Care seeks a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan. You will work in a team-based setting focused on prevention, diagnosis, treatment, and education.

You’ll conduct physical exams, diagnose and manage conditions, and perform in-scope procedures while collaborating with ENT, Surgical, and Medical subspecialties. Documentation is via E-ClinicalWorks; a strong patient-connection and teamwork are essential.

Competenze

• Certified NP-C/FNP or PA-C licensed in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Broad patient panel across all ages and backgrounds.
• Proficient in outpatient clinical procedures.
• Strong communication and empathy; connect with patients.

• UtilizeE-ClinicalWorksfor efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills- your ability toconnectwith patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
