Job summary

Elara Caring is seeking an Occupational Therapist to join our team and help deliver compassionate home health care. You will evaluate patients, develop treatment plans, educate families, and coordinate with physicians to support independence and daily living skills.

The role requires an active OT license, graduation from an accredited OT program, and at least one year of clinical experience. Travel to patient homes is expected, so a valid driver’s license or reliable transportation is required.

Qualifications

• Current, unrestricted OT license valid for the state of work.
• Graduate of an accredited Occupational Therapy program.
• Minimum one year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients
• Must possess a valid driver's license or reliable transportation to travel between patient homes

Responsibilities

• Educate patients and caregivers on home therapy programs, adaptive techniques, and equipment.
• Monitor patient progress and adjust treatment plans based on changing needs and physician orders.
• Communicate patient progress, changes in condition, and safety concerns to physicians and the care team.
• Coordinate adaptive equipment and specialized devices, including custom splints when needed.
• Provide guidance and supervision to COTAs, Home Health Aides, and other clinical staff involved in patient care.
