Job summary

Elara Caring is seeking a dedicated Physical Therapist to provide in-home care and contribute to our mission of delivering Right Care, at the Right Time, in the Right Place. You will evaluate patients, develop personalized treatment plans, and deliver therapy in the home setting while collaborating with physicians and the care team.

Requirements include an APTA-approved degree, an active PT license in the work state, at least 1 year of healthcare facility experience, and a valid driver’s license

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

What is Required?

• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
