Job summary

The Kids Clinic in Delhi is seeking a Medical Transcriptionist to convert dictated medical content into precise reports and letters. The role requires careful review, consistent style, and translation of abbreviations to full terms.

You will maintain confidentiality, ensure accuracy, and update patient summaries, discharge notes, and investigations in the software, coordinating with healthcare staff as needed.

Responsibilities

• Transcribe and interpret the dictation into diagnostic test results, operative reports, referral letters and other documents
• Review and edit drafts, making sure that the transcription is correct, complete and has a consistent style
• Translate medical abbreviations and jargon into the appropriate long form
• Identify inconsistencies, errors and missing information within a report that could compromise patient care
• Follow up with the healthcare provider to ensure the accuracy of the reports
• Submit health records for approval
• Follow patient confidentiality guidelines and legal documentation requirements
• To update the admitted patients summary
• To handover the Discharge summary to the respective staff nurse before the patient is discharged.
• Enter the investigations done or services given for the In-patients in the activity card, put sign and seal once it is updated in software.
• To sign and put seal on the No-due certificate only after cross checking the system entries with the activity card.
• Communicate with concerned staff nurse/Doctors/ Lab or radiology dept/ other dept. for any queries in services or investigation.
• To prepare different kinds of letters (Approval letters, Medical Certificates, Estimation letters etc).
• Death Summaries to be updated in-time and has to be cross checked and signed by the respective consultant or Assistants.
• Case summaries have to be made for the corporate patients, in case of prolonged admission.
• Maintain Discharge TAT records and monthly update to HOD
