Job summary

Medanta in Gurugram seeks a Medical Transcriptionist to accurately transcribe doctor dictations, consultation notes, discharge summaries and medical reports, upholding high accuracy and confidentiality.

The role requires 30 WPM typing, strong medical terminology knowledge, anatomy understanding, and proficiency with HIS/EMR and MS Office. Prior hospital experience in healthcare documentation is preferred.

Qualifications

• Graduate in any discipline; certification/course in Medical Transcription or Healthcare Documentation preferred.
• 1–4 years of experience in medical transcription or healthcare documentation.
• Typing speed of 30 WPM or above is mandatory.
• Working knowledge of human anatomy, medical tests and healthcare terminology is essential.
• Good English grammar, spelling and proofreading skills.

Responsibilities

• Transcribe doctor dictations, consultation notes, discharge summaries, operative notes and medical reports accurately.
• Ensure correct use of medical terminology, diagnoses, procedures and clinical abbreviations.
• Proofread and verify all transcribed documents for accuracy, completeness, grammar and formatting.
• Maintain defined TAT and productivity standards for assigned transcription work.
• Coordinate with doctors/clinical teams for clarification of unclear or incomplete dictations.
• Review medical reports for missing or inconsistent information.
• Upload and maintain completed documents in the HIS/EMR/Medical Records system.

Job Objective

To accurately transcribe medical dictations, clinical notes, discharge summaries, investigation reports and other medical documentation while maintaining high standards of accuracy, confidentiality and timely completion.

Job Responsibilities

• Transcribe doctor dictations, consultation notes, discharge summaries, operative notes, procedure notes and medical reports accurately.
• Ensure correct use of medical terminology, diagnoses, procedures, medications and clinical abbreviations.
• Proofread and verify all transcribed documents for accuracy, completeness, grammar and formatting.
• Maintain the defined TAT and productivity standards for assigned transcription work.
• Coordinate with doctors/clinical teams for clarification of unclear or incomplete dictations.
• Review medical reports and documentation for missing or inconsistent information.
• Maintain confidentiality and security of patient information at all times.
• Upload and maintain completed documents in the HIS/EMR/Medical Records system.
• Ensure proper documentation and filing of medical records as per hospital standards.
• Support timely completion of discharge documentation and discharge summaries.
• Follow hospital policies, SOPs, NABH requirements and medical record documentation standards.
• Maintain daily productivity and error/rework reports.
• Perform any other duties assigned by the reporting manager.

Key Skills & Competencies

• Minimum typing speed: 30 WPM with good accuracy.
