Job summary

Centre For Sight in Vadodara is looking for a meticulous Medical Transcriptionist / Data Entry Operator to join its ophthalmology team. You will prepare OPD notes, discharge summaries, and other documentation, while entering patient data into HIS/EMR and maintaining strict confidentiality.

The role calls for fast, accurate typing, solid medical terminology knowledge, and proficiency with MS Office. You will support audits, generate reports, and help ensure timely, precise documentation.

Qualifications

• Typing speed 40–60 WPM with high accuracy.
• Strong knowledge of medical terminology, ophthalmology terms and abbreviations.
• Proficiency in MS Office, HIS/EMR systems.
• Excellent attention to detail and proofreading skills.
• Ability to maintain confidentiality and meet documentation timelines.

Responsibilities

• Prepare OPD notes, discharge summaries, operative notes, referral letters, and diagnostic reports.
• Enter patient demographics, clinical findings, diagnoses, investigations, prescriptions, and treatment details into the HIS/EMR.
• Maintain confidentiality and security of patient information in accordance with hospital policies.
• Scan, upload, and organize medical records, investigation reports and consent forms.
• Generate daily, weekly, and monthly documentation and MIS reports as required.
• Support audits by ensuring complete and accurate medical documentation.

Medical Transcriptionist / Data Entry Operator

Key Responsibilities

• Prepare OPD notes, discharge summaries, operative notes, referral letters, and diagnostic reports.
• Enter patient demographics, clinical findings, diagnoses, investigations, prescriptions, and treatment details into the Hospital Information System (HIS) or Electronic Medical Record (EMR).
• Maintain confidentiality and security of patient information in accordance with hospital policies.
• Scan, upload, and organize medical records, investigation reports and consent forms.
• Generate daily, weekly, and monthly documentation and MIS reports as required.
• Support audits by ensuring complete and accurate medical documentation.

Required Skills

• Strong typing speed (4060 WPM preferred) with high accuracy.
• Good knowledge of medical terminology, anatomy, ophthalmology terms, and abbreviations.
• Proficiency in MS Office, Hospital Information Systems (HIS), or Electronic Medical Records (EMR).
• Excellent attention to detail and proofreading skills.
• Ability to maintain confidentiality and meet documentation timelines.

Preferred Qualifications

• Graduate in any discipline.
• Diploma or certification in Medical Transcription is preferred.
• 0–3 years of experience in medical transcription, healthcare documentation, or hospital data entry.
• Experience in an eye hospital or multispecialty hospital is an added advantage.
