Job summary

Qualifications

• 2 years paid experience in social care (essential).
• Social Studies / Case Management Accredited Qualification.
• Understanding of risks in homeless and social care contexts.

Responsibilities

• Provide for residents' practical needs while maintaining a safe, high-standard environment.
• Assess residents' needs and refer to appropriate agencies.
• Provide key working, record progress, advocate for residents and update manager.
• Liaise with external agencies, health boards and county councils.
• Adhere to quality assurance policies and procedures.
• Assist medical and nursing staff with residents' health care.
• Participate in handovers, supervise volunteers, support team decisions.
• Engage in supervision and annual appraisals, identify own development.
• Adhere to Depaul policies, including Health & Safety, Code of Conduct and Confidentiality.
• Maintain professional standards in line with Depaul values.
