Job summary

The Rehab Group is seeking a Relief Community Support Worker in Galway. This 12-month fixed-term contract involves delivering individualized community support to people with intellectual, physical and mental health challenges and helping them to participate in local activities and programs.

You will plan and deliver educational and recreational activities, document progress, and maintain dignity, respect and independence for service users. A full driving licence is essential.

Qualifications

• QQI Level 5 in Health and Social Care or equivalent required.
• Willingness to participate in Rehab Care training and external training opportunities.
• Experience supporting individuals with intellectual, physical and mental health challenges.
• Experience helping individuals identify meaningful experiences in their local community.
• Experience promoting self-advocacy.
• Experience providing personal care support.
• Experience supporting community independence.

Responsibilities

• Plan, develop, implement and review educational, recreational and occupational programs in the community.
• Document information, update records and complete required paperwork.
• Ensure interactions with service users are respectful, dignified and empowering.
• Deliver a quality service by applying monitoring systems and assisting with daily living skills.
• Support independence by helping service users engage with local community resources.
• Maintain privacy and confidentiality of service users at all times.
• Follow health and safety regulations and carry out risk assessments.
