Job summary

Rehab Group is seeking a Relief Community Support Worker to deliver personalised support to individuals with disabilities, helping them engage in meaningful activities within their local communities. The role requires a QQI Level 5 in Health and Social Care or equivalent, willingness to undertake RehabCare training, and a full manual driving licence.

The position is a 12-month fixed-term contract based in Liosban Business Park, Tuam Rd, Galway, with a pro rata salary of €31,561.56 (€15.56/hour)

Qualifications

• Minimum QQI Level 5 in Health and Social Care or equivalent is desirable.
• Willingness to participate in Rehab Care training programme and relevant external training opportunities.
• Experience supporting individuals with intellectual, physical and mental health challenges.
• Experience promoting self-advocacy.
• Experience in the provision of personal care support.
• Experience in supporting community independence.

Responsibilities

• Plan, develop, implement, monitor and review educational, recreational and occupational programs in the community.
• Document information, update records, and complete necessary paperwork for communication and accountability.
• Ensure interaction with service users is respectful, dignified and inclusive of choice.
• Deliver a quality service by implementing and reviewing quality monitoring systems and assisting with daily living activities.
• Support service users to integrate into their local community and access local resources.
• Maintain privacy and confidentiality of service users.
• Comply with health and safety legislation and follow risk assessments.
