Qualifications

  • Associate degree from an APTA-approved PTA program.
  • Valid New York State PTA license.
  • Newly licensed PTAs welcome to apply.
  • Skilled nursing facility experience preferred.
  • Strong communication, documentation, and organizational skills.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
  • Commitment to providing compassionate, resident-centered care.

Responsibilities

  • Provide skilled physical therapy treatments under the direction of a Physical Therapist.
  • Assist with implementing individualized treatment plans to improve mobility, strength, balance, and functional independence.
  • Monitor resident progress and communicate treatment updates to the supervising Physical Therapist.
  • Complete required documentation, treatment records, and care plan updates accurately and timely.
  • Educate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment.
  • Assist with discharge planning, home exercise programs, and equipment recommendations.
  • Collaborate with physicians, nursing staff, and interdisciplinary team members to support resident outcomes.
  • Participate in staff training, in-services, and quality improvement initiatives.
  • Maintain therapy equipment, supplies, and treatment areas.
  • Perform additional duties as assigned.
