Job summary

Praxis Care is seeking a Part Time Support Worker at Metges Road for 21.5 hours per week. This role forms part of a dedicated team delivering high-quality, person-centred care, supporting independence and improving outcomes.

Salary is based on 39 hours full-time and pro rata to contracted hours; sleep-in payments (where required) are specified. A driving license and access to a vehicle are preferred.

Qualifications

• Essential: Numeracy and literacy skills.
• Essential: Effective communication skills.

Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans.
• Work closely with Team Leaders to prepare and implement individual and group support programmes.
• Collaborate with other Support Workers to promote a safe working and living environment, resolving situations using appropriate techniques and language.
• Ensure that medication is stored and administered in accordance with Praxis Care’s Medication Policy where necessary.
• Familiarize yourself with administrative requirements and log information per reporting guidelines.
• Be aware of all service policies and participate in related training.
