Job summary

Connected Health Group Limited seeks a Rapid Response Healthcare Assistant to deliver urgent, hands-on care in clients’ homes across Ireland. You will support immediate care needs, step into new packages, and help ensure safety, comfort and independence in the community.

Ideal candidates have 1+ year homecare experience, QQI Level 5 in Health & Social Care, and a valid driving licence with access to a car. Competitive salary, sign-on bonus, mileage paid, fortnightly pay, and progression

• Experience in homecare (1+ year in Ireland).
• QQI Level 5 in Health & Social Care required.
• Driving licence + car essential due to travel between clients.
• Calm under pressure due to rapid changes in care needs.
• Strong communication & organisation for juggling visits and reporting.
• Compassion and empathy core to homecare work.
• Weekend flexibility required due to rota system.

Responsibilities

• Provide rapid response care to clients in their homes.
• Support new care packages and discharges.
• Monitor and report on care quality to area manager.
• Collect and review journal notes from clients’ homes.
• Deliver care plans, logs, and PPE to client locations.
• Undertake additional reasonable duties as required.

Key Duties And Responsibilities

• To be responsible for staff shadowing - ensuring Health & Safety policies and procedures
• To undertake emergency over - Rapid response (filling new packages and discharges)
• To evaluate and monitor the quality of the care delivered to clients and report to the area manager with concerns
• Checking and collecting journal notes from clients houses
• Delivery of Careplans, log sheets and gloves to client's homes when required
• Delivery of PPE
