Job summary

Praxis Care seeks a Team Leader for Brookvale to lead a team of Support Workers and design individual support programmes. The role focuses on ensuring safe transitions, responsive care, and adherence to policy, with a person-centered approach.

Essential criteria include 2 years’ experience in health and social care and QQI Level 5 in Health Care, plus a valid driving licence. A sleep-in allowance is provided where applicable, and the post supports flexible hours to meet service needs.

Qualifications

• 2 years’ experience in a health and social care setting.
• QQI Level 5 in Health Care or equivalent.
• Right to Work in the Republic of Ireland.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service.
• Liaise with families and multi-disciplinary teams to promote needs.
• Plan, develop, and implement individual and group programmes of support.
• Coordinate ongoing assessment and reviews using suitable models.
