Praxis Care in Ireland is seeking a Team Leader to guide a team of Support Workers, design individual support programmes, and ensure safe, person-centered care for the individuals we support. The role operates through a key worker system with a focus on rights, dignity, and independence while coordinating service delivery.
Responsibilities include liaising with families and multidisciplinary teams, developing and reviewing care plans, and overseeing medication handling in line with policy.
The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service.
Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by a group of Support Workers and themselves.
All advertised salaries are based on 39 hours (FTE) and will be pro rata to contracted hours.**
