Job summary

Altius Hospitals Puri Indah is seeking a qualified Pharmacist to support ER, clinical, and logistic pharmacy operations. The role emphasizes fast, accurate dispensing for ER, involvement in clinical rounds, and inventory control to ensure drug availability.

Minimum D3/S1 with STR active; hospital pharmacy experience preferred. Must be able to work shifts, including night shifts, and coordinate with ER doctors and nurses to optimize patient care.

Qualifications

• Minimum D3/S1 Pharmacy with STR active.
• Preferably experienced in hospital pharmacy.
• Knowledge of emergency medications for ER.
• Experience in clinical pharmacy, ward rounds, or hospital inpatient.
• Experience in inventory control, FEFO/FIFO, stock opname, and CDOB standards.
• Good communication with ER doctors and nurses.
• Willing to work in shift including night shift.

Responsibilities

• Dispense medications for ER patients quickly and accurately.
• Prepare emergency drugs, IV admixtures, and high-alert medications as needed.
• Ensure medication availability for ER crash carts, emergency kits, and ambulance stock.
• Review patient medication therapy to ensure safety, efficacy, and appropriateness.
• Participate in clinical rounds with medical teams.
• Provide drug information, counseling, and education for patients.
• Monitor potential drug interactions, allergies, duplication, and dose adjustments.
• Manage daily receiving, storage, and issuing of pharmaceutical items.
• Maintain accurate inventory records and conduct regular stock opname.
• Ensure correct storage conditions (temperature, segregation, cold chain).
• Prepare internal distribution to units (inpatient, outpatient, ER, etc.).
