This is a full-time Care Coordinator role at Homecare Gurus Ltd, located in High Wycombe, with flexibility for some work from home. The Care Coordinator will be responsible for care coordination, case management, communication with clients and healthcare professionals, providing home care services, and delivering exceptional customer service.
- Care Coordination and Case Management skills
- Previous experience in Home Care services
- Ability to work independently and collaboratively
- Excellent organizational and time management skills
- Previous experience in a similar role is a plus
