Clinical responsibilities: To independently undertake specialised clinical assessments, diagnose, treat, and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses. To ensure patients are fully involved in the design and implementation of their treatment plans. To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required. To refer appropriately to clinical leads/supervising GP where a second opinion is required. Please see attached job description for further information and an example working schedule.
