Job summary

Waythrough is seeking a compassionate Complex Needs Practitioner to join the Transitions Team in Birmingham, supporting young people aged 17-25 who may experience substance misuse, mental health difficulties and other complex needs.

You will work person-centred, build trusting relationships, deliver flexible, multi-agency interventions across the community, helping young people identify goals, reduce risk and develop resilience for positive outcomes.

Qualifications

• Experience in health or social care.
• Experience working with young people.
• Ability to work independently and as part of a team.
• Empathetic, non-judgemental approach with resilience.

Responsibilities

• Support young people using a person-centred and strengths-based approach.
• Identify goals, reduce risk and develop skills and resilience for positive outcomes.
• Support those with multiple risk factors including parental substance misuse.
• Work across the community and with multi-agency partners to respond to referrals.
• Build trusting relationships and provide tailored interventions.
