Waythrough in Birmingham is seeking a Complex Needs Practitioner to support young people aged 17–25 through key transitions, including challenges from substance misuse and mental health difficulties. The role is based in Birmingham with a strong community focus.

You will work in a timely, person-centred way to identify goals, reduce risk and build resilience, collaborating with health, social care, education and criminal justice partners. Your energy and empathy will drive positive outcomes.

Qualifications

• Experience in health or social care.
• Ability to work with young people 17–25 with complex needs.
• Strong relationship-building with multi-agency partners.

Responsibilities

• Support young people through transitions and case management.
• Build trusting relationships with clients and partners.
• Coordinate with health, social care, education and criminal justice services.
• Deliver person-centred, strengths-based interventions.
