Job summary

Places for People is seeking a Homeless Support Worker to provide high-quality support to young people in a Supported Housing setting. You will manage caseloads, develop individual support plans, and work with external agencies to deliver coordinated services that promote independent living.

The role involves safeguarding responsibilities, accurate record-keeping, and participation in multi-agency meetings.

Qualifications

• Experience managing customer caseloads, support plans, risk assessments and safeguarding concerns.
• Experience working with external agencies and multi-disciplinary partners to deliver positive customer outcomes.
• Strong IT and administrative skills, including maintaining accurate customer records, support plans and case management systems.

Responsibilities

• Deliver high-quality support to customers who may have complex and enduring support needs.
• Manage a caseload of young people and support them to achieve independent living and housing outcomes.
• Coordinate with external agencies and partners; attend multi-agency meetings to provide updates on progress.
• Maintain accurate records and ensure safeguarding concerns are reported in line with policies.
