Job summary

Confidential is seeking a Care Coordinator to plan, deliver, record and review each young person’s care in line with care plans and regulatory requirements. You will be the main point of coordination between placement teams, Placement Manager and external professionals.

You will ensure placements are organised, compliant and focused on the young person’s needs, providing updates to managers to keep an accurate placement picture.

Qualifications

• Experience in health & social care administration or care coordination.
• Strong organisational and administrative skills.
• Confident communicator (written and verbal).
• Competent with IT systems and digital care platforms.

Responsibilities

• Coordinate day-to-day running of placements, including routines, appointments and transport.
• Attend LAC reviews, professionals' meetings and multi-agency meetings.
• Provide timely updates, reports and incident notifications to placing authorities.
• Support governance, record-keeping and compliance with CQC & GDPR.
