Job overview

We are seeking an enthusiastic and highly motivated Diabetes Specialist Nurse (DSN) to join the Barts Health NHS Trust Young Adults Diabetes Service, which is a mobile service comprising a multidisciplinary team including a Dietician, Clinical Psychologist, Youth Worker, Social Prescriber, and administrator.

This new role aims to improve diabetes care and outcomes for young people aged 16-25 across North East London. The postholder will support young adults managing various difficulties related to living with diabetes, especially during their transition to adult services. The work involves supporting both Type 1 and Type 2 diabetes patients.

The post offers scope for service development with support from senior colleagues and collaboration with service users. The base will be Mile End Hospital, but work will span across all Barts Health sites, ensuring standardized, high-quality care.

We welcome applications from qualified diabetes specialist nurses interested in working with this patient group. The successful candidate will be part of the core young adult MDT and the wider Barts Health diabetes service, receiving support from experienced clinicians.

Main duties of the job

The role involves joining an innovative service focused on improving diabetes care and outcomes for young people (16-25) with both Type 1 and Type 2 diabetes across Barts Health. The postholder will support high-risk young adults, working with social workers, youth workers, and psychologists to safeguard and engage these patients.

The postholder will develop specialist skills to provide management plans, treatment advice, and support for young adults and their carers. The role involves working across all sites, providing specialist care, and supporting young people, their families, and clinical teams. The postholder will serve as a clinical and educational resource, always striving to improve care and be a team player.
