We are seeking a motivated Diabetes Specialist Nurse (DSN) to join the Young Adults Diabetes Service at Barts Health NHS Trust. This mobile service includes a multidisciplinary team comprising a Dietician, Clinical Psychologist, Youth Worker, Social Prescriber, and administrator.
This role aims to improve diabetes care and outcomes for young people aged 16-25 across North East London, supporting those struggling with living with diabetes and transitioning to adult services. The work involves supporting young adults with Type 1 and Type 2 diabetes.
The post offers scope for service development, with support from senior colleagues and collaboration with service users.
The base is at Mile End Hospital, but work spans all Barts Health sites, ensuring standardized, high-quality care.
We welcome applications from qualified diabetes specialist nurses interested in working with this patient group.
The successful candidate will join the core young adult MDT and the wider diabetes service, supported by experienced clinicians.
The role involves joining an innovative service focused on improving diabetes care for young people (Type 1 and Type 2) across Barts Health. You will support high-risk young adults, working with social workers, youth workers, and psychologists to safeguard and engage patients.
You will develop specialist skills to provide management plans, treatment advice, and support for young adults and their carers.
You will work across all sites, providing specialist care and support to young people, their families, and clinical teams.
You will serve as a clinical and educational resource for patients and staff, always striving to improve diabetes care.
