Job summary
An opportunity has arisen for an enthusiastic and highly motivated Diabetes Specialist Nurse (DSN) to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. This mobile service includes a Dietitian, Clinical Psychologist, Youth Worker, Social Prescriber, and Administrator.
This new post aims to improve diabetes care and outcomes for young people aged 16-25 across North East London. The post holder will support young adults facing difficulties related to living with diabetes, particularly during their transition to adult services. The work involves supporting individuals with both Type 1 and Type 2 diabetes.
The post offers scope for service development, with support from senior colleagues and collaboration with service users.
The base will be at Mile End Hospital, with work across all Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), ensuring standardized, high-quality care across the Trust.

As part of the core young adult multidisciplinary team (MDT), the successful candidate will join the wider Barts Health diabetes service and be supported by experienced clinicians.
This role is an exciting opportunity to join an innovative service aiming to improve diabetes care and outcomes in young people (both Type 1 and Type 2) across Barts Health. The post holder will be part of a core Young Adults diabetes team responsible for diabetes transition and young adult services across Barts Health.
The primary role involves supporting high-risk young adults, working with social workers, youth workers, and psychologists to safeguard and engage these patients.
The post holder will develop specialist skills to provide management plans, treatment advice, and support for young adults (16-25) with diabetes and their carers.
They will work across all sites, providing specialist care and support for young people, their families, and site-based clinical diabetes teams.
The post holder will serve as a clinical and educational resource for patients and healthcare professionals and will be a proactive team player committed to improving diabetes care.
