The post holder will have responsibility to provide a single point of access for all referrals into Adult Mental Health services.
To review all referrals to identify gaps in the process and strengthen joint partnership systems to improve early and appropriate referrals.
To collate and analyse statistical data from the current database to facilitate the process.
To triage all referral information, making autonomous clinical judgments to prioritise and allocate referrals to appropriate professionals within our service or suggest alternatives to the referrer. This is in line with the Mental Health Triage Scale, risk management processes, the Mental Health Measure (Wales) 2010, and UK Mental Health Triage Scale Guidelines (2015).
To triage phone calls from external agencies and the public, providing brief interventions during calls.
To conduct holistic assessments and formulate risk assessments.
