Dorset HealthCare University NHS Foundation Trust is seeking a part-time Rehabilitation Assistant to join the Ward Therapy multidisciplinary team in Bridport. You will support adult patients, help with functional assessments, and assist the Therapy team to enable safe discharge from hospital.

The role offers a supportive learning environment with opportunities for development and various internal apprenticeships. A DBS check is required and driving is desirable.

Responsibilities

• Support adult patients within Bridport Community Hospital to promote independence and safer discharge.
• Carry out functional assessments as directed by the Therapy team.
• Follow therapy intervention plans to improve quality of life.
• Provide admin support to ensure efficient day-to-day running of the service.
• Communicate effectively with MDT including nurses, therapists and family members.
