Job summary

South Warwickshire NHS Foundation Trust is seeking an experienced nurse to join the iSPA Integrated Care Co-ordination Service (ICC) at Heathcote House with Agile Homeworking. The role focuses on triaging urgent referrals, prioritising patient safety, and coordinating access to community and hospital services.

You will use clinical knowledge and risk assessment skills to triage and allocate visits to patients in the community, while supporting primary and secondary care collaboration.

Qualifications

• Experienced nurse with sound clinical knowledge base.
• Strong risk assessment skills for clinical triage.
• Ability to triage urgent referrals and coordinate care across out-of-hospital services.

Responsibilities

• Triages urgent referrals using clinical knowledge and CAD portal.
• Refer, allocate and support access to out-of-hospital services, primary and secondary care.
• Coordinate resources within Out of Hospital services to maximise access to care.
• Ensure all contacts with clinical triage services reach effective outcomes for patients.
• Provide managerial support to the team including supervision and recruitment as needed.
