Leeds Community Healthcare NHS Trust is seeking a skilled Triage Clinician to join our Community Nursing team in Leeds. You will perform clinical triage, prioritise referrals, provide timely advice and safety-netting, and coordinate workloads to support safe, effective patient care in the community.

You will work closely with Neighbourhood Team colleagues, GPs, and other partners to manage both planned and unplanned care pathways, ensuring patients receive the right care at the right time.

• Clinically triage referrals to community nursing services, prioritising according to clinical need and risk.
• Provide timely clinical advice, signposting, and safety-netting to patients, carers, and professionals.
• Allocate and coordinate workloads within the community nursing team.
• Escalate concerns appropriately and support urgent or complex cases.
• Maintain accurate and contemporaneous clinical records.
• Work collaboratively within the multidisciplinary team to support seamless patient care.
