Job summary

Birmingham Community Healthcare NHS Foundation Trust is seeking an experienced Band 7 Senior Clinical Practitioner to join our Temporary Staffing Department as the Bank Single Point of Access.

Based at Priestley Wharf, you will provide advanced clinical leadership, undertake tele-triage, assessment and care planning for patients with acute, undiagnosed or long-term conditions to prevent hospital admissions and support timely discharge.

Qualifications

• Registered healthcare professional with UK registration.
• Experience in triage and care planning.
• Prescribing independently where required.
• Ability to lead clinical teams and supervise colleagues.

Responsibilities

• Provide safe telephone triage and advanced clinical assessment.
• Make direct referrals and prescribe where appropriate.
• Coordinate discharge and support admission avoidance.
• Lead, supervise and support colleagues in a multidisciplinary team.
