Job summary

EPUT is seeking Band 6 practitioners for the Urgent Community Response Team in West Essex. You will assess and manage acutely unwell patients at home, using respiratory, heart failure and frailty pathways to prevent hospital admission. Remote tech and new ways of working Support early discharge with high-quality community care.

You will be part of an expanding service, flexible in hours, with opportunities for professional development, supervision and a strong multidisciplinary team.

Qualifications

  • Registered nurse or Paramedic with Band 6 level experience.
  • Experience delivering care in the community setting.
  • Able to travel between sites and conduct home visits.
  • Mentorship qualification and continuing professional development.

Responsibilities

  • Prioritise and plan own workload with some direction.
  • Provide autonomous patient care using advanced knowledge.
  • Negotiate expectations with patient, carer, and colleagues.
  • Ensure assessments and care plans meet clinical standards.
  • Deliver home visits and discharge planning within protocols.
  • Support safe discharge and communicate with GPs.
