Job summary

Procare is recruiting a Band 6 Senior Nurse for Urgent Community Response to deliver high-quality nursing care in adults’ homes across Guildford and Waverley. You will perform holistic assessments, coordinate care with GPs and other services, and contribute to reducing hospital admissions through a proactive home-based approach.

The role emphasises medicines management, wound care, catheter care and palliative support, with opportunities for leadership, professional development and mentorship

Qualifications

• Registered Level 1 Nurse with current NMC registration.
• Band 6 experience in the community setting.
• Practice Supervisor and Assessor (to be completed within 1yr of post)

Responsibilities

• Provide high-quality, patient-centred nursing care in patients' homes.
• Perform holistic assessments and clinical interventions.
• Respond to urgent referrals and prioritise visits by clinical need.
• Manage medicines, wound care, catheter care and palliative/end-of-life care.
• Keep accurate electronic records using EMIS and coordinate with GPs and community services.
• Support service improvement, clinical audit and supervise junior staff and students.
