Job summary

Provide is a Community Interest Company delivering home-based health and social care in Essex. We seek a Nursing Associate/Practitioner to join our Urgent Community Response Team, delivering autonomous, high-quality patient care in patients' homes and co-ordinating with the wider team.

The role involves clinical assessments, venepuncture, catheter care, fall support and management of acute illness, with full training provided and a 7-day service operation.

Qualifications

  • Foundation Degree is required.
  • NMC Pin is required.
  • Community or acute experience is essential.

Responsibilities

  • Provide support to the UCRT team including Nurses and Allied Health Professionals.
  • Carry out comprehensive assessments in patients' own homes.
  • Implement and evaluate individual care plans under supervision.
  • Maintain clear communication with patients, relatives and the team.
  • Plan for equipment needs and logistics for home visits.
