Job summary

East Suffolk and North Essex NHS Foundation Trust is seeking a Nurse Practitioner to deliver urgent community care through the UCRS. You will undertake rapid assessments within two hours of referral and manage urgent presentations in patients' homes and care homes.

You will prescribe when appropriate and provide short-term support to prevent hospital admission, while collaborating with GPs and community teams to ensure safe, high-quality patient care. A car driver is essential for field visits.
