Job summary

City Health Care Partnership CIC is seeking an Urgent Care Practitioner to join our team in Goole, delivering timely assessment and treatment for patients with minor injuries, minor illnesses, and acute health concerns. You will work with a multidisciplinary team, ensuring safe care, appropriate referral, and opportunities for ongoing learning and development.

You will assess, diagnose, and treat within your scope, refer appropriately, and contribute to evidence-based practice and service

Qualifications

• Registered clinician with experience managing minor injuries/illness
• Strong clinical assessment, decision-making and communication skills
• Ability to work within professional boundaries while delivering patient-centred care
• Commitment to high standards, teamwork and continuous learning
• Experience in acute or primary care setting
• Experience of workload management and meeting deadlines
• Experience with SystmOne and email tools

Responsibilities

• Assess, examine and manage patients with undiagnosed or acute health needs
• Diagnose and treat minor injuries and illnesses within scope
• Refer patients to ensure safe ongoing care and onward referral
• Support evidence-based practice, clinical audit and service improvement
• Act as a positive role model and provide guidance to junior colleagues
