Job summary

IC24 is seeking an UCCH Coordinator to help coordinate timely community-based urgent care, supporting multidisciplinary teams and partners to avoid unnecessary hospital admissions.

You’ll manage referrals, monitor demand, maintain records, and contribute to service improvements while working across a dynamic rota including evenings and weekends. This role emphasizes patient-focused care and safeguarding.

Qualifications

• Experience working within NHS, Integrated Urgent Care (IUC) or a healthcare setting.
• Highly organized with the ability to prioritise competing tasks.
• Remain calm under pressure and adapt quickly to changing situations.

Responsibilities

• Receive and process requests for assistance from healthcare professionals and other service partners, directing patients to the most appropriate community-based care.
• Provide coordination and administrative support to multidisciplinary teams to deliver timely responses to UCCH referrals.
• Monitor service queues and support fluctuating workloads to maintain service performance and patient access.
• Build effective relationships with healthcare professionals and partner organisations for clear information sharing.
• Record and manage patient and service information across operational systems, ensuring accuracy and compliance.
• Contribute to quality improvement initiatives and support new ways of working to enhance patient outcomes.
• Recognise safeguarding concerns and support patient safety and wellbeing at all times.
• Provide cross-team support, including training and contingency planning during rota-based service needs.
