We and have an exciting opportunity for an urgent care practitioner to join our well-established and high-performing group of Practices.

The role will involve working as part of a team with other members of staff (including other urgent care practitioners and a Duty Doctor) to help our patients access any urgent 'on the day' support they may need, including daily visits to patients' homes.

The post is permanent and full time although we do have some flexibility regarding hours and may consider part-time applicants.

Main duties of the job

Working as part of our Practice Team you will provide care topatients from initial history taking and clinical assessment through todiagnosis, treatment, prescribing and evaluation of care.

You will be confident carrying out clinical triage andface-to-face consultations using your knowledge to investigate, diagnose,treat and refer registered patients within set guidelines.

As an advancedclinical practitioner we are looking for the following experience:

A relevant Nurse Practitioner or Paramedic Qualification toMasters Level or equivalent.

Previous minor illness experience.

Job responsibilities

To support the daily on call team to provide anon the day emergency service for patients. The role will involve theassessment, diagnosis, treatment and/or referral of patients presenting withundifferentiated and undiagnosed conditions. The post holder will hold aprescribing qualification and will work within their level of competency.

·To provide assessment, diagnosis and treatment at first point of contact byattending to patients autonomously, within the GP surgery, their own homes andnursing or residential homes.

  • To support andcontinue the development of on the day urgent patient service in thePractice.
  • To accuratelytriage and prioritise patients, including in acute situations,demonstrating the use of a variety of techniques to elicit the history ofan event/illness, including past medical and drug history.
  • To evaluateclinical information from examination and history taking, and initiateappropriate treatment and/or referral.
  • To undertake andinterpret diagnostics and take appropriate action to manage patient care,whilst distinguishing between abnormal and normal findings in order torelate them to a possible diagnosis. To use broad clinical guidelines(NICE etc) relating them to individual cases, providing assistance andfeedback to the organisation and others on how these should beinterpreted.
  • To signpostappropriately to other services in the identified healthcare pathway.
  • To refer patientsto appropriate care settings, within locally agreed healthcare pathways.
  • To discharge ortreat, as appropriate and within the scope of practice.
  • To maintainaccurate and systematic documentation of patient interactions and clinicalcare provided, including adverse events.
  • To advise andsupport members of the public on healthy lifestyles, health promotion anddisease prevention.
  • To provideclinical advice to other members of the team where appropriate.

  • A relevant Paramedic Qualification

  • Ability to adapt to service needs whilst working in a busy environment.
