Telford and Wrekin Council is recruiting a Triage Officer to join the Making Prevention Real pilot, supporting the front door of Adult Social Care. You will gather information, coordinate referrals and help with timely decisions to link residents to the right services.

The role offers 37 hours per week on a fixed-term basis for 12 months with a hybrid working model and opportunities for development. DBS checks may apply.

Responsabilidades

• Gather information from individuals, carers, health professionals and partner organisations to support timely decision‑making.
• Coordinate referrals to ensure people receive appropriate advice, support and intervention.
• Support the multi-disciplinary team in delivering short‑term, targeted support to stabilise or resolve presenting needs.
• Contribute to a preventative, strengths‑based approach that promotes independence and wellbeing.
• Play a key role in ensuring a seamless journey through Adult Social Care and efficient connection to services.
