Job summary

Telford & Wrekin Council is recruiting two experienced Occupational Therapists to deliver the Making Prevention Real pilot at the front door of Adult Social Care. You will work in a multidisciplinary team, focusing on prevention, early intervention and enabling independence for residents.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Experience in moving and handling and proportionate care.
• Ability to work in a multidisciplinary triage setting and promote prevention.

Responsibilities

• Deliver Making Prevention Real pilot services at the front door of Adult Social Care.
• Lead occupational therapy practice in a multidisciplinary team and advocate strengths-based approaches.
• Assess and identify care needs, enablement and timely referrals while ensuring least restrictive interventions.
