Job summary

The Dudley Group NHS Foundation Trust is seeking a Community Nurse Practitioner for a 27.5 hour per week post. You will assess health needs autonomously, diagnose, treat and prescribe where required, and refer or discharge as appropriate within a dynamic community setting.

You will join the Community Urgent Care Response Team, delivering high-quality, patient-centered care and contributing to service development under the supervision of Advanced Clinical Practitioners and Medical Leads.

Qualifications

• Current NMC registration and first level registered nurse.
• Education to degree level or equivalent experience.
• Independent/Supplementary Nurse Prescriber (V300).
• Evidence of ongoing professional development and clinical governance understanding.
• Ability to work autonomously with advanced clinical assessment and decision-making.

Responsibilities

• Provide and support high-quality patient care in the community.
• Conduct rapid clinical assessments for undifferentiated conditions with evidence-based plans.
• Deliver care within the Community Urgent Care Response Team and meet national 2-hour targets.
• Lead and contribute to service development and clinical hub activities.
