Job summary
Oxford Health NHS Foundation Trust is recruiting a Centralised Triage Lead to develop and lead a centralised triage across District Nursing services. The role provides operational and clinical leadership to ensure referrals are prioritised and allocated according to patient need and risk, supporting integrated working across community services.

Qualifications
- Provides clinical and operational leadership for the Centralised Triage Service.
- Develops triage pathways, SOPs and referral management processes.
- Oversees referrals prioritisation per service criteria and risk frameworks.
- Monitors demand, capacity and workforce to maximise service effectiveness.
- Supports governance, quality assurance and patient safety initiatives.
Responsibilities
- Lead and develop staff through supervision, appraisal and professional development.
- Act as clinical expert resource for complex referrals and escalation decisions.
- Work with District Nursing Teams and partner organisations to ensure integrated working.
- Use data to identify trends, risks and opportunities for improvement.
- Drive service redesign and transformation projects related to access, assessment and referrals.
