Job summary

The Dudley Group NHS Foundation Trust is recruiting a Community Nurse Practitioner to deliver high-quality patient care using advanced clinical practice and professional judgement to assess, diagnose, treat and discharge patients where appropriate.

You will work autonomously within the Community Urgent Care Response Team, delivering rapid assessments for undifferentiated conditions and contributing to the development of the Clinical Hub, while supporting flexible working and NHS People Promise

Responsibilities

• Assess health and clinical needs of patients autonomously.
• Diagnose, treat and prescribe where required,Refer to other services and discharge patients.
• Make rapid clinical assessments for undifferentiated conditions within the 2-hour care target.
• Contribute to ongoing service development and clinical leadership within the hub.
