Job summary

Moxie People in Newport is seeking an Outreach Support Worker to deliver trauma-informed, person-centred support within a Psychologically Informed Environment (PIE). You will assess needs, develop action plans, collaborate with housing teams, health services, and police, and help clients achieve independence.

This role offers a temporary contract with varied day shifts. Applicants should have sector knowledge, strong communication and IT skills, safeguarding experience, and be willing to travel

Qualifications

• Experience addressing the needs of homeless and vulnerable clients.
• Strong written and verbal communication; proficient IT skills.
• Ability to manage a busy workload, pay attention to detail and make decisions under pressure.
• Commitment to safeguarding, compassion, integrity and teamwork.
• Must be a driver for the outreach element across Newport.
• DBS on the update service is essential for a quick start.

Responsibilities

• Conduct initial assessments and risk management plans using a trauma-informed approach.
• Develop and regularly review tailored client support plans to maximise entitlements.
• Liaise with Local Authority housing teams, health services, and police for holistic care.
• Facilitate client engagement meetings and help design in-house support programmes.
• Maintain accurate real-time client records and contribute to KPIs.
• Delegate tasks to Assistant Support Workers and participate in duty rotas.
