Job summary

Supply Desk Ltd. is assisting a welcoming primary school in Greenford to appoint a Transition Teacher for full-time work starting in January.

The role focuses on supporting pupils during key transitions between year groups, with a strong emphasis on continuity in learning and emotional support. The ideal candidate will have experience in primary settings (EYFS/KS1/KS2), strong classroom management, and the ability to collaborate with teachers to monitor progress and tailor interventions.

Qualifications

• QTS is desirable
• Experience teaching in primary settings (EYFS, KS1, KS2)
• Strong understanding of the primary curriculum and progression between key stages
• Excellent classroom management and organisational skills
• Ability to work full-time in the Beaconsfield area from September

Responsibilities

• Plan and deliver engaging, differentiated lessons to support pupils across transition phases
• Provide targeted support for pupils adapting to new curriculum and classroom environments
• Work with class teachers to ensure continuity in learning and assessment
• Support pupils’ academic development in reading, writing, and maths
• Foster confidence, resilience and independence during periods of change
• Identify and support pupils needing additional intervention, including those with SEN
• Monitor and assess progress, using data to inform support strategies
• Build positive relationships with pupils, staff and parents to ensure a smooth transition process
• Contribute to a positive, inclusive school environment
