Job summary

The Rowans Hospice in Portsmouth and South-East Hampshire is seeking a Transition Nurse to lead the transition pathway within Living Well Services. This role supports adults and families facing life-limiting illness, across two shifts per week (15 hours).

You will work within a multidisciplinary team, develop MDTs, improve referral pathways and produce accessible care documents for professionals and families, based at the Living Well Services Centre, Purbrook.

Qualifications

• Excellent interpersonal, verbal and written communication skills.
• Understanding of grief and bereavement counselling.
• Ability to manage sensitive information and work in a busy, emotional environment.

Responsibilities

• Re-establish and develop Transition MDT meetings.
• Develop a shared care framework and referral pathways.
• Map and integrate best-practice transition models into local pathways.
• Produce clear pathway documents for professionals and families.
