Job summary

The Rowans Hospice in Portsmouth and South-East Hampshire is seeking a Transition Nurse to lead the transition pathway within Living Well Services. This role supports adults and families facing life-limiting illness, across two shifts per week (15 hours).

You will work within a multidisciplinary team, develop MDTs, improve referral pathways and produce accessible care documents for professionals and families, based at the Living Well Services Centre, Purbrook.

Qualifications

  • Excellent interpersonal, verbal and written communication skills.
  • Understanding of grief and bereavement counselling.
  • Ability to manage sensitive information and work in a busy, emotional environment.

Responsibilities

Transition Nurse - AHP Project

£39,959.00 - £48,117.00 FTE depending on experience

15 hours per week to be worked across two shifts a week.

Rowans Hospice is a charity that provides free care and support to adults and their families living in the Portsmouth and South‐East Hampshire region who have a life‐limiting illness. Established in 1994, our income is generated by ongoing support of the local community, donations, legacies, fundraising and through our retail shops.

The Role

The Transition Nurse/AHP role will work within the Living Well Services, which are led by the Living Well Services and Community Engagement Manager who are responsible to the Clinical Lead. The role is based at Rowans Living Well Services Centre, Purbrook.

Key Working Relationships

Internal: Living Well Services Staff, Specialist palliative Care Team

External: Staff from Jacks Place and Chestnut Tree Children’s Hospice. Transition Links within Hospices – using networks. Key links with those supporting transition in the community

Key Areas of Responsibility / Key Accountabilities
Re‑establish and Develop Transition MDT Meetings
  • Rebuild MDT Transition Meetings with CTH and develop separate MDT with NH/JP
  • Cross‑Service Representation
  • Case Discussion and Clinical Support Forum
  • Support the existing joint initiative, now including JP.
Develop a Shared Care Framework
  • Create a Shared Care Protocol
  • Improve Referral Pathways into RH
  • Map existing transition pathways used by other specialties.
  • Adapt and integrate best‑practice models into a localised palliative care transition pathway.
  • Produce a clear, accessible pathway document for professionals and families.
Experience, Qualifications, Skills and Behaviours required
  • Excellent interpersonal skills and the ability to interact well with the team, clients, volunteers and the bereaved.
  • Excellent verbal and written communication skills and the ability to prepare comprehensive notes.
  • Understanding of grief and bereavement counselling.
  • Decision making, being responsible for knowing appropriate limitations of the role and when to seek further advice.
  • Able to manage unexpected disruption or changes in your daily routine or schedule.
  • Able to handle high‐pressured and emotional situations in a busy environment.
  • Ability to manage sensitive and confidential information.
  • Able to show effective time management and diligence skills.
  • Strong analytical and data‐inputting skills and ability to manage multiple tasks at a time.
  • Leadership insight with the evidence of effective working with outside agencies and teams
  • Ability to prioritise work load and be flexible to changing demands
  • Ability to share knowledge with others to develop and influence practice
