Job summary

HCRG Care Group in Surrey is seeking a compassionate Transition Lead Nurse to develop and deliver an integrated transition programme for young people moving from children’s to adult services. You will review current arrangements, design pathways, SOPs and tools across partner organisations, and chair multi-agency groups to drive service improvement.

Applicants must be a Registered Nurse (NMC), hold a Nursing degree, provide evidence of CPD, and have a full UK driving licence.

Qualifications

• Significant experience working with children, young people or adults with complex health needs.
• Experience leading service development or quality improvement projects.
• Experience of multi-agency and partnership working.
• Experience developing policies, pathways, protocols or operating procedures.
• Experience influencing change across organisational boundaries.

Responsibilities

• Lead the development and delivery of an integrated transition programme to improve outcomes for young people moving from children's to adult services.
• Review current transition arrangements, identify opportunities for improvement, and develop standardised pathways aligned with national guidance and best practice.
• Design, implement and embed transition pathways, policies, SOPs, and supporting tools across partner organisations.
• Collaborate with young people, families, carers and professionals to ensure transition services are person-centred and inclusive.
• Build partnerships across health, social care, education, and voluntary sectors to drive service improvement.
• Chair and facilitate multi-agency stakeholder groups with leadership and coordination on transition best practice.
• Develop and monitor performance measures, analyse outcomes and implement quality improvements.
• Deliver training, guidance and resources to support practitioners.
