Job summary

Chelmsford College is seeking an Inclusion Coach – Transition Lead (Term Time) to shape and strengthen transition pathways for learners with SEND across the College. You will coordinate high‑quality, inclusive transition support as learners move between programmes and progress to positive destinations, working with curriculum teams, support services and external partners.

Qualifications

• Experience supporting learners with SEND in an educational setting is desirable.
• Strong commitment to inclusion and improving outcomes for learners.

Responsibilities

• Lead and coordinate transition arrangements for learners with SEND across entry, progression, and exit routes.
• Design and deliver tailored transition programmes for learners with higher needs or vulnerable cohorts.
• Organise and oversee transition activities and tasters.
• Track learner progress and transition data to inform timely interventions.
• Collaborate with SENCO, SEND Team Leader, Inclusion Coaches and academic teams to support progression.
