An exciting opportunity has arisen to join the Healthcare Transition Team at Sheffield Children’s Hospital. This role will support the development and delivery of high-quality, patient-focused care for young people requiring transitional care across a range of specialties.
The successful candidate will provide comprehensive administrative support to the team, including organising and coordinating meetings, taking and distributing meeting minutes, managing diaries, and undertaking a variety of other administrative duties as required.
The Healthcare Transition Team is a small, friendly, and supportive central team based on the main acute site at Sheffield Children’s Hospital.
Working across the Trust and all care groups, the team provides expert guidance and support to specialties throughout the transition process, helping to ensure young people experience a smooth and effective transition into adult services.
This role is responsible for providing administrative and clerical support to the Healthcare Transition team
• The post holder will aid the team in the completion of specific tasks associated with the delivery of Healthcare Transition.
• The position requires sound knowledge of IT systems, excellent communication skills with staff at various levels in the organisation and will necessitate hands‑on use of information systems.
• The post holder must establish good working relationships and clear communications with colleagues. These working relationships will be at every level including senior clinicians and managers.

This is a significant senior clinical leadership opportunity at Dementia UK. You will lead our national Admiral Nurse Helpline and Clinics services, with accountability for their quality, performance and continued development as we grow the reach and impact of our specialist dementia support.
As Head of Helpline and Clinics, you will provide operational and professional leadership across a large, highly experienced remote team of Admiral Nurses working throughout the UK.Leading four Deputy Leads and a wider team of around 70 nurses, you will be accountable for services delivered through our award-winning Helpline, Live Chat and clinic services.
You will be a registered nurse with substantial experience of clinical operational management and leading large, complex clinical teams. You will understand what it takes to run high-quality, responsive, remote services at scale, balancing workforce planning, resources, budgets, governance and day‑day operational demands while keeping the needs of families at the heart of decisions.
We are recruiting multiple Admiral Nurse roles to join our Helpline and Clinics team to expand ourTo be successful in this position, you will need to be an active Registered Nurse on the NMC register with a post‑registration qualification or training in dementia care or a relevant subject. You will have significant experience of practicing as an Admiral Nurse or dementia specialist nurse and previously worked with people who have dementia and their family carers and supporters in different settings.
Strong communication skills are essential, as is a proven track record of providing high standards of clinical leadership and specialist nursing support. You should be adept at providing clinical advice, clinical supervision, and advocating for family carers.
