Job summary

Royal National Orthopaedic Hospital NHS Trust in London is seeking a clinician to undertake clinical triage of therapy referrals to the Referral Hub, deciding on the correct care pathway for each patient. You will carry a clinical caseload to support community rehabilitation and work collaboratively within a multidisciplinary team.

You will partner with Referral Hub staff and management to ensure timely allocation of patients to the correct teams.

Qualifications

• Undertake clinical triage of therapy referrals to the Referral Hub and decide on the correct care pathway for each patient.
• Carry clinical caseload to support community rehabilitation.
• Work collaboratively within a multidisciplinary team.

Responsibilities

• Attend liaison with acute hospitals, local authorities and other stakeholders to obtain information and resolve queries.
• Participate in ongoing monitoring and evaluation of the therapy triage, ensuring data collection and undertaking audits.
• Provide an evidence-based assessment, treatment and advisory service to clients with a variety of conditions.
